The following will be requested upon completing the application and once the student is admitted. The administration will be in contact with what forms are needed to complete registration.

Please download and print the following documents upon request from our administration’s office and return signed from your physician.

• Physician’s Report Form
• Immunization History Form
• Vaccination Excemption Form
• Allergy Action Plan Form
• Medical/Medicine Administration Form
• Intent to Homeschool Form *Although we are an exempt private school, parents should fill this out just case if you are pulling your students from CCSD.
